Rural residents who commonly seek treatment for advanced stages of colorectal cancer have been thought to lack access to cancer screening and adjuvant therapy. ... Several theories have been postulated to explain these findings, some of which include low socioeconomic status, lower educational attainment, lack of insurance coverage, underinsurance, and travel distance to health care facilities. ... Colonoscopy is the most sensitive and specific test to screen for colorectal cancer in individuals older than 50 years. ... A rural-urban disparity exists in the density of gastroenterologists, general surgeons, and radiation oncologists who traditionally provide colorectal cancer screening services and treatment. This might affect access to these services and may negatively influence outcomes for colorectal cancer in rural areas (Aboagye, Kaiser and Hayanga, 4/16). On a national level, the provision of samples with a prescription by dermatologists has been increasing over time, and this increase is correlated with the use of the branded generic drugs promoted by these samples. Branded and branded generic drugs comprised most of the prescriptions written nationally (79%), while they represented only 17% at an academic medical center clinic without samples. Because of the increased use of branded and branded generic drugs, the national mean total retail cost of prescriptions at an office visit for acne was conservatively estimated to be 2 times higher (approximately $465 nationally vs $200 at an academic medical center without samples) (Hurley, Stafford and Lane, 4/16). This analysis examines reference pricing, a form of defined contribution health benefits, where plan sponsors pay a fixed amount or limit their contributions toward the cost of a specific health care service, and health plan members must pay the difference in price if a more costly health care provider or service is selected. Reference pricing for hip and knee replacement, colonoscopy, magnetic resonance imaging (MRI) of the spine, computerized tomography (CT) scan of the head or brain, nuclear stress test of the heart, and echocardiogram were examined because these services have fairly uniform protocols .... Potential aggregate savings could reach $9.4 billion if all employers adopted reference pricing for the health care services examined in this paper (Fronstin and Roebuck, 4/15). States are increasingly turning to Medicaid managed care as a key strategy to manage costs and encourage innovation in health care delivery. This report examines health care providers' perspectives on the role of managed care in improving health services for low-income adults in four communities: Milwaukee, Wisconsin; Oakland, California; Seattle, Washington; and Washington, D.C. It finds that providers do not generally perceive Medicaid managed care as a catalyst for delivery system reform. Fragmented delivery systems, limits on the types of services for which managed care organizations are at risk, and the volatility in managed care markets all present challenges to improving care delivery (Summer and Hoadley, 4/14). A marked and growing interest in shifting to a defined contribution approach for both pre-65 and post-65 retiree coverage is fueled by the employers desire to manage future costs. Increasing interest in moving from group coverage to non-group coverage is a trend that is particularly strong with respect to Medicare-eligible retirees for whom employers can facilitate access to non-group coverage through private exchanges. ... the new federal/state marketplaces are gaining at least the consideration by employers as a possible pathway through which the employer's pre-65 retiree population might gain access to non-group coverage (McArdle, Neuman and Huang, 4/14). Over the last decade, New Hampshire has pioneered health care price transparency to support cost-conscious consumer behavior and, ultimately, to spur competition and increase efficiency among health care providers. In 2003, the state mandated one of the nation's first all-payer claims databases .... Using panel data from a large hospital system, this paper presents estimates of the productivity effects of human capital in a team production environment. Proxying nurses' general human capital by education and their unit-specific human capital by experience on the nursing unit, we find that greater amounts of both types of human capital significantly improve patient outcomes. Disruptions to team functioning attributable to the departure of experienced nurses, the absorption of new hires, and the inclusion of temporary contract nurses are associated with significant decreases in productivity beyond those attributable to changes in nurses' skill and experience (Bartel et al., April 2014). Eric L. +A0 Eric L. Every since I wrote the column "Crabs in a Barrel," the gross hypocrisy of South Carolina State Senator, Rev. Darrell Jackson, has lingered with me. In that column I describe how the Black, South Carolina State Senator, who was also the pastor of the 10,000 member, "Family Way Bible Life Center Church," came out against Senator Barack Obama during the 2008 presidential race after selling his support to Senator Hillary Clinton for $10,000 a month--a dollar amount that corresponded exactly with the number of members he had in his church. Now, as I drive through the Black community and see the magnificent "houses of God" towering over the community in the midst of poverty, social need, and in some cases, squalor all around them, I can't help but wonder how many of them are really doing God's work. When I look at those resplendent edifices, it takes me back to a little storefront church on 108th and Juniper in Watts, where my grandmother first sent me for religious instruction. She was ill at the time, so she couldn't take me to "the big church" in which she was a member. I'll never forget that little storefront church. It stood down the street and in the shadow of a huge and elaborately appointed Catholic church, seemingly, almost as an afterthought. It was so small and had so few members that the thought of attracting a true "ordained man of God" was out of the question, so we had to settle for a little, unassuming man that we used to refer to as Elder Hampton. That little church was the closest thing to worshiping in someone's living room as you could get. But to this day, whenever I begin to lose faith in the basic goodness of my fellow man, or even remotely begin to contemplate God, I think of that little storefront church. If I'd remained at that church, I don't know what I would have been doing today. My young instincts led me to become so close to Elder Hampton that I might have even become a preacher. He used to take me with him to visit the old, the poor, and the sickly in the neighborhood. Black, Mexican, young, old, Baptist or Catholic, he didn't care what a person was—if they were sick or in need, they were all a part of his flock. Looking back on it, I don't know where he found the resources. He certainly didn't get it from our little collection plate. We were so poor and so few in number he couldn't have gotten more than ten dollars a Sunday out of us, max. But in spite of that, he was no Sunday preacher. He was a full-time man of God--if you were sick or in need, you could count on him seven days a week. After my grandmother had an operation and finally got over her illness, they took me to the "big church." I never saw Elder Hampton again. His influence has remained constant in my life to this day—in fact, though I must admit that I'm rarely found in church these days, it is his lingering influence that's led me to write this article, and everything else that I write about. The first time I went to the big church I was completely overwhelmed with the opulence of it all. Shortly before I arrived, the church had just imported in a new fireball of an ordained minister, direct from Dallas, Texas. He was nothing like the quiet and humble Elder Hampton. He had a big booming voice, wore shinny Florsheims, expensive suits, and a sense of importance just oozed from every pore of his body. When this man walked into a room it sucked all the oxygen out of the place. You just knew you were in the presence of someone significant. God must have loved this magnificent church the minister headed. The choir alone in this ornate house of God was larger by a factor of three than the entire membership of the little church I'd grown accustomed to. The choir pit was twice as large as the room where we held Sunday school. The parking lot of the church was filled with big expensive cars and a limousine was often parked next to the front entrance. In addition, City Councilmen and other politicians were counted among its membership, and a well known entertainer was the church organist. In a church like this you didn't have to wait to get to heaven--every Sunday you were right there. The only problem was, after services you had to return to reality, which was more often than not, a life of pure hell. I've often wondered what Elder Hampton would have done with all of those resources. But I couldn't imagine Elder Hampton heading a magnificent place like this. Since he wasn't ordained, he never would have even been considered to head a place like this in the first place. I don't care how Godly you were, stature took precedence there–if Moses wanted to head the church, they'd want to inspect his credentials. But even if Elder Hampton would have been ordained, he was much too humble a man to be embraced by the high-powered people in this membership. With his quiet, unassuming demeanor, even as a member, he would have been a back-bencher—politely tolerated, but scarcely noticed. What's more, Elder Hampton wouldn't begin to know how to manage the resources of a huge church like that. While he was definitely a man of God, he wasn't a practical man. He probably would have squandered all the church's resources on the no-account sinners in the surrounding community. He was just too impractical. He probably would have set up a soup kitchen and built a dormitory to house the homeless at night, and hired unemployed mothers to start a low-cost daycare center on the church grounds during the day. Then it wouldn't be no time at all before he'd dig up the church's beautiful grounds, trying to put up basketball courts and a recreation building to draw young people off the streets after school. And of course, between the kids during the day, the teenagers in the early evening, and the homeless at night, it would cost the church a fortune just to keep up the grounds and repair damages to the building. Much of the congregation would have been in an uproar over the chaos he'd create. And it would go far beyond simply chaos. He would have long since lost most of the influential politicians. Every Sunday he'd be hounding them at church and calling them at home during the week, trying to get them to create Empowerment Zones, and Special Need Zones to establish low-cost loans to help the surrounding neighbors to purchase and fix-up the houses they were living in. It's no wonder he'd drive the politicians out of the church. No, Elder Hampton's heart was in the right place. But he'd have been much too impractical to run a big church like that. He was a God loving man, but he lacked common sense. He thought when God said Love thy neighbor, he meant it. What a fool. Yeah, Elder Hampton would have been a back-bencher at that church. He would have been the fool sitting way in the back--with Jesus. “Beware of practicing your righteousness before other people in order to be seen by them, for then you will have no reward from your Father who is in heaven. When you give to the needy, sound no trumpet before you, as the hypocrites do in the synagogues and in the streets, that they may be praised by others. Truly, I say to you, they have received their reward. But when you give to the needy, do not let your left hand know what your right hand is doing, so that your giving may be in secret. And your Father who sees in secret will reward you. “And when you pray, you must not be like the hypocrites. For they love to stand and pray in the synagogues and at the street corners, that they may be seen by others. Truly, I say to you, they have received their reward . . . The results are in and the winners of the Most Cost-Efficient Primary Election Victor award — an award I just made up — are Carol Ammons, Pat Quinn (sorta) and Jim Oberweis (for real). An electoral candidate gets this prestigious award by a) winning the primary and b) spending as little money as possible. The first hurdle is important because otherwise this year's award-winners would have been 13th Congressional District Republican Michael Firsching, who got 2,145 votes without spending any money. (At least he didn't report any spending to the Federal Election Commission). Other cost-efficient losers were Tio Hardiman, the Democrat who got 123,078 votes against Quinn (34 cents for each vote); Doug Truax, the Republican U.S. Senate candidate against Oberweis ($1.16 for each of his 326,948 votes); 13th District Democrat David Green ($1.83 for each of 4,431 votes); Republican gubernatorial candidate Kirk Dillard ($7.92 for each of his 302,035 votes); and 13th District Republican Erika Harold ($12.43 for each of the 20,921 votes she got). The two least cost-efficient losers — a real double whammy — were 13th District Democrat George Gollin of Champaign ($50.05 for each of his 9,908 votes) and Democratic Illinois House candidate Sam Rosenberg of Champaign ($51.55 for each of the 2,511 votes he received). On the other hand Bruce Rauner and Rodney Davis won, but they spent a lot of money doing it. Rauner spent $47.16 per vote ($15.24 million for 323,296 votes) and Davis spent $43.40 ($1.205 million for 27,773 votes). The winners, therefore, are Democratic Illinois House nominee Ammons of Urbana (who unlike Quinn and Oberweis had a legitimate opponent who outspent her) who spent $14.23 for each vote she got; Democratic Gov. Quinn ($4.68 per vote) and Republican U.S. Senate nominee Oberweis ($1.47 per vote). The latter is significant because Oberweis had a reputation for wildly spending and losing anyway. In the 2006 Republican gubernatorial primary, for example, he spent $15.64 per vote. There's a chance the old Oberweis could return this year when he takes on Democrat Dick Durbin, a race he has said will require millions of dollars. An early poll shows Durbin with a 24 percentage point lead. The Yes for Independent Maps group — the remarkably well-funded organization that is pushing for a state constitutional amendment that would allow for an independent citizens' redistricting commission to draw state legislative district boundaries — reported spending $1.285 million in the January through March quarter. Of that sum an equally remarkable $1.062 million went to NW Democracy, a Portland, Ore. Michael Kolenc, manager of the Yes for Independent Maps group, said the petition-passers are not paid for each signature they get. "They are paid $14 an hour if they work over 30 hours a week. Petition gatherers are staff and not contractors and are trained on a daily basis," Kolenc said in an emailed response to questions. "They report to one of our four offices in Chicago, the collar counties or Rockford. In Champaign County, though, a corps of volunteers has gathered more than 12,000 signatures on "Yes" petitions. "We have been clear from the beginning that collecting this many signatures would require hundreds of staff collecting on a daily basis," Kolenc said. "A large amount of our signatures are collected by our 28 regional volunteer teams, an infrastructure that will be around after signatures are submitted to Springfield. Trent Shepard of Urbana, who said he has collected at least 1,500 signatures on petitions, said he didn't resent that he did his work for free while others were paid. "From the beginning they were clear with us that this is what had to be done," Shepard said. "It's sad to me that this couldn't all be done with volunteers but I guess they knew what they were talking about. He noted that the campaign's coordinators reported recently that volunteers had gathered only about 75,000 of the reported 450,000 signatures expected to be collected. "So if that's all that volunteers alone would have gathered this would have been another failure," Shepard said. State Sen. Chapin Rose, R-Mahomet, said he felt the same as Shepard. "It's the reality of getting to that huge number," he said. "You're going to need both. You're going to need them to pull in those big numbers. A large majority of their numbers will be valid. But you're going to need the volunteer ones that are going to have a high degree of validity to get them over the top when it comes to the petition challenge. Yes for Independent Maps reported $1.87 million in campaign contributions during the three-month period ending March 30. Last week a rumor popped up — I got three calls about it within an hour — that retiring state Rep. Naomi Jakobsson, D-Urbana, was interested in becoming the Democratic candidate for Champaign County clerk to oppose the currently unopposed Gordy Hulten. Nope, said Jakobsson. "I hope they haven't started passing petitions," Jakobsson joked. "I have no plans to run for county clerk. I do not plan to run for clerk. Jakobsson, 72, who served 12 years as county recorder and is completing her 12th year in the Legislature, said she is looking forward to retirement. "I fully want to be able to spend time with my family and to do some volunteer work in the community," said Jakobsson. (Will you not gift a victory to your sister)," Renuka said. The Congress nominee is hoping to sway the high percentage of minority voters at Haridwar in her favour. Of the 15.53 lakh voters, the constituency has about 3.85 lakh Muslim voters and 1.85 lakh SC/ST voters. Nishank, however, told TOI, "People will not be misled by meaningless slogans and tag lines like Double ka Dum or buy one and get one free." The BJP leader pointed out that after winning in 2009, Harish rarely came to the constituency and it was only after being made the CM that he realized his duty towards Haridwar as he did not want the party ticket to go outside his family. "The question still remains that what has the couple done for the religious city apart from making hollow commitments," he added. Upbeat after the recent meetings in minority-dominated areas of Haridwar, Nishank said, "Both minorities as well as SC/STs are backing BJP this time and we will register a thumping win. Adding to Renuka's problems, former CM and MLA of Doiwala assembly under Haridwar, Nishank, hit the campaign trail about a month before her name was announced as Congress nominee. It was only after Harish took oath as Uttarakhand CM on February 1 that the name of his family members, including Renuka and daughter Anupama, cropped up for the seat. Congress won only four seats while BSP emerged victorious in three constituencies. Police attended a disturbance at an Ellenbrook house at about 10.10pm last night, where a man was damaging the house and a 16-year-old girl had been seriously assaulted. The Ellenbrook home belonged to the man’s former mother-in-law, where his ex-partner and their daughter were also staying. It will be alleged the man was invited over for the night, but started to drink and become agitated. East Metropolitan District Office Acting Superintendent Steve Byrne said it appeared the family, including five children, ran outside to escape the man but he chased after them. It will be alleged he then started to bash his ex-partner’s 16-year-old niece with a baseball bat on the front lawn, inflicting injuries on her arms and wrists, while his former partner ran back inside. Police say he then bashed down the front door with the bat and when he could not find his ex-partner, he grabbed the three-year-old girl and drove off in a Kia Carnival. At about 11pm, the car was spotted being driven along Lloyd Street in Midland without headlights on. Police attempted to stop the vehicle, but the driver failed to stop. Because of the child in the car, officers did not start a pursuit but did monitor the movements of the car. It was found on fire a short time later after it had crashed into a concrete barrier on Great Eastern Highway near the Roe Highway on-ramp. It will be alleged the man had already left the scene on foot, leaving his daughter inside the burning wreck. Acting Supt. Byrne said he believed the child was in the front seat of the car and not in a proper child seat. Officers from Midland Police Station managed to rescue the child, using fire extinguishers to allow them to get her, and the fire flared again during the rescue. The girl was taken to Princess Margaret Hospital by ambulance with serious injuries — including a tear to her bladder. She had to have emergency surgery to stop internal bleeding and is in a spine splint as a precautionary measure. She remains in a serious but stable condition. The man was found 150m away from the crash scene hiding behind a vehicle on Beaconsfield Avenue, Midvale, where he was arrested and taken to Swan District Hospital by St John Ambulance for treatment to injuries he received in the crash. Acting Supt. Byrne said he found the situation “abhorrent”. “The violence inflicted on the family by this gentleman is unfathomable,” he said. “To leave a child like that, to flee, for your own concern and not have any concern for your own child, I just can’t imagine. Acting Supt. Byrne said he had been told the man was upset over the recent death of a brother in a car accident. ■ driving with a blood-alcohol level in excess 0. The man was refused bail and will appear in the Perth Magistrates Court today, Monday April 21. He was returned to hospital and may require a bedside hearing. Children enjoy reading books at a book fair at Van Mieu (Temple of Literature) in Ha Noi as part of the first Viet Nam Book Day. Authorised by PM Nguyen Tan Dung, deputy minister of Communications and Information Truong Minh Tuan announced the decision, stating that April 21 every year will be marked as Viet Nam Book Day. The decision also states that the annual event seeks to encourage and promote reading habits in the community and to raise public awareness about the reading's significance in accumulating knowledge, as well as in the formation and shaping of the human personality. In addition, the day also aims to highlight the important role and position of books in the country's cultural life, as well as to honour people involving in all stages of book publication, from writing, printing, publishing, distribution, and libraries. Speaking at the ceremony, Minister of the Communications and Information Nguyen Bac Son reaffirmed that Book Day is an outstanding cultural event for book lovers and for the entire nation. "The annual organising of Book Day will boost the reading culture, thus forming a learning society in the country. "Despite society's continuous development, leading people to have abundant reading choices on e-libraries or the internet, printed books still have an irreplaceable role, which has remained for over thousands of years. Until today, books still are a valuable spiritual source," he said. Explaining the reason why April 21 has been chosen as Book Day, the minister said the day has a great significance as 87 years ago in April the book Duong Cach Menh (Revolutionary Road) by Nguyen Ai Quoc (President Ho Chi Minh) was released, encouraging the entire nation to stand up to fight for independence. Son added that Viet Nam Book Day also reflects the country's international integration into World Book and Copyright Day, which is held two days later on April 23 every year. Following a launching ceremony for Book Day at the Van Mieu (Temple of Literature) yesterday, other activities, such as book presenting, discussions on reading habits, and children's story telling, were also held. At the National Library, a book fair entitled, From the Past to Nowadays, has opened to the public until Saturday, April 26. Among the highlight of the fair is the display of a bronze book featuring the writings of Emperor Minh Mang (1791-1841) of the Nguyen dynasty. The book has been borrowed from a library in Germany. The ongoing fair will also host talks with famous writer Nguyen Nhat Anh and historical researchers Duong Trung Quoc and Le Van Lan. At the Centre for East-West Cultures and Languages, 62 Nguyen Chi Thanh Street, the ongoing display, Hanh Trinh Cua Sach Quoc Ngu Viet Nam (The Journey of the Romanised Vietnamese Books), showcasing some 200 rare books printed between 1870 and 1975, will remain open until Saturday. All the books were borrowed from private collections of members of the book forum sachxua.net. Thousands of young people in HCM City took part in a "Reading Day" that began at the Youth Cultural House on Saturday. The five-day event is organised by the Departments of Information and Communications and Culture, Sports and Tourism along with the HCM City Youth Union to mark Viet Nam Book Day. Le Thai Hy, director of the Department of Information and Communications, said the festival is aimed at encouraging and promoting the habit of reading among people. Fourteen major publishers and distributors, including Tre (Youth) Publishing House, Tong Hop Publishing House, and the HCM City Book Distribution Company (FAHASA), are exhibiting their latest and best-selling books for visitors to read for free. They also offer discounts of up to 20 per cent for people buying them. Nguyen Cam Tu, a student at Open University, said: "I love reading. But as a student I cannot afford to buy too many books, so the event is a chance for me to enjoy reading. I can find all kinds of books from reference to literature. There will be seminars on developing the reading habit among children and youths. Scholars and educators like writer Do Hong Ngoc, Nguyen Tan Thanh Truc of Smart Libraries project, and Le Truong Tung, headmaster of FPT University, spoke at the event. There is an exhibition of books related to Hoang Sa and Truong Sa and the victory over the French at Dien Bien Phu in 1954. The City's Youth Union is doing a year-long survey to discover the 100 books liked the most by young people and children. Lam Dinh Thang, deputy secretary of the union, said the survey aims to identify the books that had the greatest impact on the lives of young people and children and promote a reading culture among them. The survey will end on December 31. According to Hy, the Reading Day is the city's third big event this year to promote reading after the Book Street Festival during the Lunar New Year in January and the Book Fair last month.
